We are currently focusing our energy on America’s chronically-ill, aged or disabled dually eligible population – those with both Medicare and Medicaid coverage – to demonstrate the efficacy of our new care delivery model.

Medicare and Medicaid cover 135 million Americans who spend $1.2 trillion dollars annually. Of the 135 million Americans, 11 million (9%) have both Medicare and Medicaid (dually-eligible) benefits and yet consume 33% of all Federal healthcare spending. That is $450 billion dollars. Why? It is primarily due to poor management of chronic conditions in their homes. Other factors include difficulty navigating our healthcare system; inability to get proper medical, emotional, and family support; nutritional issues and social isolation.

We have a passionate desire to actively improve health equity for these often-marginalized populations. By demonstrating the success of our unique care delivery model, our goal is to help drive legislative changes to the Medicare and Medicaid programs creating an integrated benefit for the dually eligible, driving a better quality of life for beneficiaries, reducing the burden on the Medicare and Medicaid programs, and lowering costs to the American taxpayer.
