This Privacy Policy describes how PHC Buyer, LLC (“CRH”) and its affiliates, as applicable (together referred to as “Company” or “we” or “us” or “our”), collects, uses, and shares information about people who visit us online at www.CareRingHealth.com (the “Site”), and who communicate with us in other ways (“you” or “your”).

By visiting our Site or communicating with us in other ways, you agree to the collection, use, and processing of your personal information as described in the Privacy Policy. If you do not agree to the terms of this Privacy Policy, please do not access our Site or provide your personal information to us.
Scope of This Privacy Policy
This Privacy Policy describes how Company collects, uses, shares, and protects your personal information and your choices regarding use, access, and correction of your personal information. We use the term “personal information” in this Privacy Policy to refer to information that identifies, relates to, describes, or is reasonably capable of being associated or linked, directly or indirectly, to you.
We collect personal information that you submit to us directly, and we may collect personal information that is gathered automatically online by cookies and similar internet technologies, and that we receive from social media and other sources. For example, we may collect information about you when you:

Collection by third party websites. Our Site may contain links to third-party websites that we do not operate or control. Anonymized information. Information that has been modified in a substantial way so that it no longer can reasonably identify or be linked to an individual.
Business to business communications. Information we obtain about our employees, job applicants, and other personnel or the employees and representatives of business customers, service providers, suppliers, and business partners.
Personal Information You Provide To Us
Initial Information. When you initially provide information to us, and depending on the purpose for which you provide information to us, we ask for your:

- E-mail address
- Phone number
- Best time of day to contact you
- Insurance coverage type
- Age range (for prospective patients)
- How you heard about us
- Reason for inquiry
- Information for employment application purposes, which may include the position you are interested in, name, address, cell phone number, e-mail address, the service location you are interested in, an option to upload your resume, how you heard about us, and a field for questions
Surveys. If you participate in a survey, we may ask for your opinions and thoughts and may also ask you to provide your name, contact information, and other personal information.
Automated Online Data Collection
We (and our third-party service providers) may place cookies, web beacons, scripts, and similar tracking technologies and device identifiers on your web browser or mobile device when you visit our Site. (In this Privacy Policy we use the term “cookies” to refer generally to cookies, tracking technologies, and other internet tools). Features and performance—to provide functionality for the features and options you select to use on our Site; deliver consistent performance; recognize and implement your preferences; and enable information sharing with applicable third-party providers.
Location—If you visit our Site using a mobile device, we may obtain geolocation data from device sensors, GPS signals, WiFi access points, and cell towers, unless you disable location information in your device setting. We can also more generally identify your location through IP address or mobile device identifier.
Data analytics—track and log web usage data about your interactions with our Site and online activities including, for example, browsing history, usage rates, settings preferences, content interactions (including searches, page views, downloads, and prints), and user journey history (including clickstreams and page navigation, URLs, timestamps, content viewed or searched for, page interactions, and similar data).
Advertising—Cookies used for advertising purposes may be set through our Site by us or our advertising partners. We and our advertising partners may use cookies to build a profile of your interests and show you relevant advertisements on our Site and on other websites and mobile applications. Advertising cookies do not store directly personal information, but store data that may uniquely identify your browser and internet device. E-mail Interconnectivity—If you receive e-mail from us, we may use internet tools to capture data related to when you open our message and click on any links or banners it contained. We may use this information to track the effectiveness of our communications with you.
Personal Information We Collect From Other Sources
SSocial Media Platforms—You may be able to connect to our Site using your social media accounts, such as Instagram, Twitter, Facebook, LinkedIn, or TikTok. You may be able to log into our Site using single sign-on services such as Facebook Connect or an Open ID. The sign-on services will authenticate your identity, provide you with the option to share certain personal information (such as your name and e-mail address) with us, and pre-populate our sign-up form. Services like Facebook Connect give you the option to post information about Company on your profile page to share with others within your network. If you authorize us to connect with you using your social media account, we may access and store your name, e-mail address, location, profile picture, other images you post, URL, and any other personal information that the social media platform makes available to us. We may use and disclose that information in accordance with this Privacy Policy.
How We Use Personal Information We Collect
We use personal information to conduct our business, stay in contact with our customers, and meet their expectations, including to:
- Send transactional messages, including responses to your comments, questions, and requests; provide customer service and support; and send you technical notices, updates, security alerts, and support and administrative messages;
- Provide, operate, maintain, improve, and promote the Site;

How You Can Restrict Interest-Based Advertising
Interest-based advertising is the collection of data from different sources and across different platforms in order to predict an individual’s preferences or interest and to deliver to that individual – or their computer, smart phone, or tablet – advertising based on their assumed preference or interest inferred from the collection of data pertaining to that individual or others who may have a similar profile or similar interests.
We work with a variety of third parties to attempt to understand the profiles of the individuals who are most likely to be interested in our services so that we can send them promotional e-mails or serve our advertisements to them on the websites and mobile apps of other entities.

These features will opt a visitor out of many – but not all – of the interest-based advertising activities in which we or third parties engage.
How You Can Restrict Cookies
Cookies enable Company to measure and analyze the visits to the Site and collect data on the activities of users of the Site and our online portal. We may use Google Analytics and/or other third-party data analytics services to place the cookies, track Site usage data, and provide reports to us. Company does not disclose any directly identifying personal information to a third-party data analytics service. However, the service may monitor a user’s visits to the Site and combine this data with data from other websites monitored by the same service provider and then use the aggregated findings for its own commercial purposes. Do Not Track (DNT) is an optional browser setting that is used by some website operators and third parties. Our Site does not respond to DNT signals.
Your Communications Choices
E-mail marketing and advertising. You can opt-out of receiving marketing communications from us by following the “unsubscribe” instructions included in each of our e-mail marketing communications. You can also opt-out by e-mailing us at info@careringhealth.com and providing the name(s)/e-mail address(es) that you want us to remove from our marketing database and add to our “Do Not Contact” list. If you opt-out of marketing messages, we may still send e-mails to you that involve your orders or responses to questions. It may take up to ten days to process your request to unsubscribe from e-mail marketing messages.
Text messages. If you previously gave us permission to send recurring automated promotional and personalized marketing communications to you by text message, you can withdraw that consent at any time by replying STOP to our text message. It may take up to seven days to process your request to STOP text marketing messages.
Retention and Protection of Personal Information
We will retain your personal information as long as we provide services to you, and as necessary to comply with our legal obligations, resolve any disputes, and enforce our agreements.
We maintain commercially reasonable security procedures and practices appropriate to the nature of the categories and specific items of personal information we collect. However, neither people nor systems can operate in a fully secure manner and no method of transmission over the Internet or method of electronic data storage is fully secure. While we use reasonable efforts to protect your personal information and select third-party service providers who we believe also use reasonable precautions, we cannot guarantee the security of the personal information you provide to us.
If you have reason to believe that your interaction with us is no longer secure (for example, if you feel that the security of any of your information has been compromised), please immediately notify us of the problem.
Links to Third-party Feature and Websites
The Site may contain links to features or websites operated by third parties. Company is not responsible for the privacy practices of any third-party services. Because these third parties will have access to your personal information, we recommend that you review their privacy policies and practices.
Third-Party Features. Our Site may use other companies’ applications to power some features. Some of these third-party features may be branded with Company logo, even though they are not operated or controlled by us. These third-party applications are able to access and collect data from your use of these features.

Processing of Personal Information in the United States
Company operates under United States laws and regulations. If you reside or are located in a different country, the data protection laws of that country may differ from the laws of the United States in the ways your personal information is protected. We want you to understand that when you provide your personal information to us, or engage in a transaction with us, you are giving your consent to (1) the transfer of your personal information outside of your country of residence, and (2) the processing of your information in accordance with the laws of the United States and the guidelines of this Privacy Policy.
Changes to this Privacy Policy
We reserve the right to amend this Privacy Policy at any time in order to address Company’s future developments, changes in industry trends, or changes in legal requirements. If we materially alter the terms of this Privacy Policy, we will post the revised Privacy Policy on the Site and provide other notification as required by applicable law.
